This legitimize any transfer of properties or assets one could leave behind. This is also required should the spouse left behind wants to get married again.
Procedures for securing a Death Certificate
Procedures for securing a Death Certificate
- Secure an application form from the Civil Registrar. Please be ready to supply the following requirement:
- Full name of the deceased
- Date of death
- Proceed to the local cashier for the payment of fees
- Present your receipt and application form to the civil registrar
- Death Certificate will then be issued immediately.
- Secure a certified true copy of the following(any two):
- Affidavit of two disinterested persons
- Death certificate issued by the church
- Present the above documents to the Mun. Civil Registrar's Office
- Secure and fill-up form
- Pay the fee at the cashier
- Posting will be for 10 days
